Update My Record
- Assisting adoption constellation members to add a letter, email, contact information change, or digital photo to an existing adoption record.
- A service for domestic or international adult adoptees (age 18+), birth parents, and adoptive parents whose adoptions were completed by Children’s Home, LSS or contracted organization.
- Records may be updated annually.

Document Request
- Assisting members of the adoption constellation receive legally allowable information from their adoption record.
- Review relevant Minnesota Statutes
- A service for international or domestic adult adoptees (age 18+), birth parents and adoptive parents requesting a document(s) from their record. This service is available to those whose adoptions were completed by Children’s Home, LSS or contracted organization.
- Document retrieval may help with the finalization and/or citizenship process.
- Practice standards have continued to evolve and change throughout our 150+ year history. Information may need to be redacted, according to applicable Minnesota Statutes.
- You can expect:
- Initial consultation with our Post Adoption staff
- Location and retrieval of original record
- Delivery of documentation
- Connection to optional continued support and resources
- Fee: $50 per request
- If fees are a barrier, you are welcome to apply for a partial or full grant through our Adoption Support Fund (application included with Service Request Form).
100+ Year Record Request
- Assisting any individual to obtain a 100+ year adoption record, as Minnesota Statute allows.
- A service available for adoptions completed by Children’s Home, LSS or contracted organization.
- Please note that the documentation varies. Practice standards have continued to evolve and change throughout our 150+ year history. Records will not be redacted, as allowed by Minnesota Statute.
- You can expect:
- Consultation with our Post Adoption staff
- Location, retrieval and delicate scanning of original adoption record.
- Delivery of electronic adoption record
- Connection to optional continued support and resources
- Fee: $100
- If fees are a barrier, you are welcome to apply for a partial or full grant through our Adoption Support Fund (application included with Service Request Form).
Certified Court Ordered Record Request
- Assistance for any individual possessing a court order to release an adoption record.
- A service available for adoptions that were completed by Children’s Home, LSS or contracted organization.
- Please note that the documentation varies. Practice standards have continued to evolve and change throughout our 150+ year history. Adoption records will be redacted, according to applicable Minnesota Statute.
- You can expect:
- To submit an applicable certified court order
- Consultation with our Post Adoption staff to discuss your individual situation
- Our Post Adoption staff will locate, retrieve and scan the adoption record
- The adoption record will be reviewed and redacted, as required by applicable statute
- Note, this step in the process may entail CH/LSS legal consultation
- Delivery of electronic adoption record
- Connection to optional continued support and resources
- Fee: $550
- If fees are a barrier, you are welcome to apply for a partial or full grant through our Adoption Support Fund (application included with Service Request Form).
Colombian Birth Certificate Support
- Assistance obtaining a notarized Colombian Birth Certificate.
- A service for Colombian adult adoptees (age 18+) whose adoptions were completed y Children’s Home, LSS or contracted organization.
- Please note we may be able to accommodate additional special requests, please contact us about your specific situation.
- You can expect:
- Initial consultation with our Post Adoption staff
- Coordination with our Colombia Representative
- Please note additional fees will apply for the Colombian Representative portion of this request. You will receive an individualized estimate of total costs prior to the start of your service
- Closing consultation with our Post Adoption staff
- Connection to optional continued support and resources
- Fee: $50 (+ Colombian expenses)

Ethiopian Adoption DVD Lifebook Replacement
- Providing additional or replacement copies of Ethiopian Adoption DVD/Lifebooks.
- A service for Ethiopian Adult Adoptees (age 18+), or adoptive parents whose adoptions were completed by Children’s Home.
- Please note, this service is available on a case-by-case basis.
- You can expect:
- Consultation with a Post Adoption staff
- Location and retrieval of U.S. and Ethiopian adoption records
- Contracted support in Ethiopia, if needed
- Delivery of replacement DVD and/or Lifebook
- Connection to optional continued support and resources
- Fee: $100
- If fees are a barrier, you are welcome to apply for a partial or full grant through our Adoption Support Fund (application included with Service Request Form).
International Adoption Finalization & Citizenship Support
- Assistance finalizing an international adoption in the U.S., after your adoption case is closed. This service may also be helpful for those obtaining Certificates of Citizenship from USCIS.
- A service for international adult adoptees (age 18+), or adoptive parents whose adoptions were completed by Children’s Home, LSS or contracted organization.
- You can expect:
- Consultation with our Post Adoption staff
- Location and retrieval of adoption record
- Support gathering and submitting required documentation for U.S. adoption petition and/or Certificate of Citizenship (COC)
- Connection to optional continued support and resources
- Fee: $100
- If fees are a barrier, you are welcome to apply for a partial or full grant through our Adoption Support Fund (application included with Service Request Form).
Non-Identifying Social Medical History Report
- Creation and delivery of a comprehensive, personalized, non-identifying social medical history report from an original adoption record.
- A service for adult adoptees (age 18+), birth parents, or adoptive parents of minors, whose adoptions were completed by Children’s Home, LSS or contracted organization.
- Practice standards have continued to evolve and change throughout our 150+ year history. Information may need to be redacted or withheld according to applicable Minnesota Statute.
- This report includes non-identifying information, if identifying information and/or connection is desired, consider our Search & Connection Service.
- You can expect:
- Initial consultation with our Post Adoption staff to discuss your individual circumstances and expectations
- Location and retrieval of adoption record
- Review and redaction of record, as applicable
- Compilation of additional information and historical context into personalized, multi-page report
- Closing consultation with our Post Adoption staff to review your report
- Connection to optional continued support and resources
- Fee: $450
